Official Sports and lots of others sell the match logs, report forms, and such. Forget that. I just made my own using MS Excel and print them using my color laser printer, though an inkjet would work just fine. Here’s a picture -

I’ve added a link in the “Referee Gear” section to download this in PDF format. The PDF file will have two cards per letter-size sheet of paper. I print them on 110# white card stock which is available from Wal-Mart or Staples. I’ve found the heavier stock easier to handle during a match because it holds up better than regular printer paper when taking it out of my pocket.

    So do you tape them together back to back? Obvious dumb question, but do you find it cumbersome that when you need the other side you have to take it out of your data wallet and flip it over? For goals for the Visiting team and for cautions?

    Why would one tape them back to back? What’s depicted above are all the fields needed for a match - with the left side for the home team and the right side for the visitors.

    And for what it’s worth, I don’t currently use a data wallet. I print these out on card stock. I fold the card in half - so that home is on one side and visitors are on the other. During the match, my yellow card is in my right hip pocket, the red card is in my right back pocket (I’m right-handed), and the match record & a pen are in my left shirt pocket.
